Waiver / Liability / Permission Form

By registering your child using our online facility you agree to the terms and conditions below. If registering at the venue you will be required to sign this Terms and Conditions document.

• You (the Parent or Care Giver) understand that Primary Sports Active will retain the enrolment information submitted on a confidential database and it shall be removed if requested. Primary Sports Active will only use this information to let you know of further programs, unless you advise us otherwise.
• You acknowledge and agree to your child, or the child you are in charge of, attending this program and that you accept the risks of your child participating in this program.
• You hereby release and discharge Primary Sports Active from any claims, which you or your children now or at any time may make in connection with or incidental to your child’s participation in the Program. Nothing in this waiver is intended to exclude anything which cannot be excluded by law.
• Primary Sports Active retains the right to exclude any further participation of any player who infringes on the rights of other participants.  
• In the event of an injury a Primary Sports Active director will phone the nominated contact person and be guided by them for medical action from that point. If the senior first aid staff director on site believes an ambulance should be called immediately, this action will be initiated.
   

Please contact the directors if you have an objection to pictures/video footage of your child appearing in publicity and advertising material for Primary Sports Active (children’s names are not used in conjunction with images).  At Primary Sports Active we specialise in providing holiday sports programs that encourage primary school boys and girls to play sport and engage in an active lifestyle.
